If a patient wishes to cancel or re-book their appointment, it must be done so within 1 full week prior to their appointment date. Failure to cancel or re-book within this time period will be subject to a non-refundable charge. Cancellations and re-bookings can only be done by speaking to our staff in person or by telephone. E-mails and voice mails will not be accepted. No exceptions.
You must have a pre-diagnosed eligible condition as assessed by a Canadian physician or specialist within the past 12 months. You must have a physician following your primary care. You must be over the age of 25.
Although you as a patient is responsible for completing your documentation, we do a review, correct, make copies of and send all documentation to the licensed commercial producer of your choice by courier. Any changes, such as an address change after your application has been approved should be brought to Cannabis Treatment Clinic’s attention and will be your responsibility.
The largest delay is usually waiting for the forms and documentation from your referring medical office, to reach over. To reach our office if you have questions or concerns please contact us.
No. We only handle MMPR applications. We can assist you however with your renewal. Other medical needs should be handled by your primary physician.
You will need to see your Family Doctor and request a referral to see a specialist. Don’t have a family Doctor? You can also do this at your local walk-in medical clinic.
While the daily grams per day amount does vary from patient to patient, our clinic’s policy is not to sign prescriptions for over 5 grams per day.
For all patients that are unable to attend a walk-in appointment, we can conduct your appointment by an in-home visit by Cannabis Treatment Clinic staff member. Your family doctor or specialist will be notified of your prescription in writing after your appointment.
If the appointment is in our clinic, it is free (covered by OHIP if you’re a resident of Ontario), however, if a home visit is required there is a fee, which it is income tax deductible and you will receive a receipt for payment of the appointment. Only some medical plans cover the cost of this medication or appointment. If you are a member of Veterans Affairs please inform our office staff at the time of booking.
You will obtain your medication from the legally licensed commercial producer that you have registered with. Prices vary from $3.00-$17.00 per gram depending on your own personal needs.

There is no age requirement for the program itself. Within our clinic we usually require you be at least 25 years of age.
All licensed commercial producers have their own approval timeline. The approval can take a few days to a few weeks, if you have questions or concerns contact us.
